Lodge Participation Flap Award Forms for Youth and Adults        Youth Participation Award Form        Adult Participation Award Form Fill out the appropriate form based on your age to document your completion of the requirements. Turn in no later than January 4th, 2009.

OA Troop Rep. Form (Download PDF Document) Register to become your unit's OA Rep.

Expense Reimbursement Form (Download PDF) If you purchase items on behalf of the Lodge, whether it is for a Lodge event, or for a Lodge Committee, you must complete this form and attach all receipts in order to be reimbursed. The PDF includes instructions on what information is required

Dues Form (Download PDF) This form is for paying your dues to the Lodge. This is a very important form to hand in because if you do not pay your small fee, you will not receive a Newsletter in the mail. Once printed out you need to fill it out and mail it to the main scout office.

Candidacy Petition Form (Download PDF) This form is a MUST if you plan to run for a Lodge Officer position. It's not too early to begin thinking about your future in The Lodge.
